Congratulations — you’re pregnant! Whether you just saw those two pink lines or you’re a few weeks into your journey, the first trimester is a whirlwind of excitement, questions, and a whole lot of changes (hello, hormones!). It’s also the perfect time to lay a strong foundation for a healthy, joyful, and well-prepared pregnancy.

As a doula who has helped hundreds of women through pregnancy, birth and postpartum, I know what moms wish they would have thought about from the beginning. To help you feel more in control and less overwhelmed, I’ve put together the ultimate first trimester checklist — packed with everything you need to do, and a few things you’ll want to do, too.

Start strong—your body’s working hard, and it’s time to support it with care, rest, and the right essentials. But, the choices you make now are going to affect the options available and experiences you have in your birth. Let’s make sure you are thinking ahead and making decisions that will set you up for success later.

Checkpoint 1: Decide Where You Will Give Birth – Start By Looking Up The C-Section Rates At Your Local Hospitals.

Where I work as a doula, there are 5 hospitals and 3 birth centers and well as home birth midwives within 45 minutes of my home – meaning I work in a lot of different settings. Some of these locations have c-section rates as high as 45% for first time moms, others as low as 10%. Many women are surprised to find out that they quality of labor and delivery wards are not the same, even in local areas. Before you choose a birth location you should look up the c-section rates for your local options. The hospitals with the lowest c-section rates typically are doing the best at keeping their mother’s healthy through pregnancy and birth and make them better options to birth in.

Checkpoint 2: Choose A Doctor or Midwife

One of the first big decisions in your pregnancy journey is choosing who will guide your care—an OB-GYN (doctor) or a midwife. This choice can shape your entire experience, from the type of care you receive to how your birth unfolds.

  • Choose a doctor (OB-GYN) if you prefer a more clinical setting, want access to advanced medical interventions, or have a high-risk pregnancy that may require specialized care. Doctors are trained for both routine and complex pregnancies and can perform surgeries like C-sections.
  • Choose a midwife if you’re aiming for a more natural, low-intervention birth and want a care provider who typically offers more time, emotional support, and a holistic approach. Midwives are ideal for healthy, low-risk pregnancies and often support births in hospitals, birth centers, or even at home.

      There are so many options for baby registries out there, but you should really pick one and stick with it. Our favorites are Amazon and Babylist. Which is better depends on your priorities. Amazon offers a simple, user-friendly experience with a huge product selection, a completion discount, free returns for up to a year, and a diaper fund option. However, it lacks customization and only allows items from Amazon.

      Babylist, meanwhile, lets you add items from any retailer, offers cash funds and group gifting for big-ticket items, and helps compare prices across stores. This flexibility is great but comes with a steeper setup, more complicated returns, and occasional customer service issues.

      If you want convenience and easy returns, Amazon is ideal. For more customization and variety, Babylist is better suited. Your choice boils down to whether you prefer simplicity or flexibility.
